Early Intervention Memory Clinic
Mental Health - Adult & Senior and Seniors Services
Provided by Providence Health Care
Through group work and individual sessions, the clinic supports people to:
- optimize brain health by reducing risks and developing beneficial lifestyle habits
- maintain functioning, emotional well-being, and quality of life
- slow down cognitive decline as much as possible
The following services may be offered based on individual needs:
- Functional and cognitive assessment by an occupational therapist
- Targeted medical assessment by a geriatrician, including recommendations shared with family physician/nurse practitioner and additional medical tests & imaging as indicated
- Individual occupational therapy for cognitive strategies and brain health habits
- Online or in-person brain health education and behaviour change groups for person with cognitive change and care partner
- Transfer to Older Adult Outpatient Clinic for more comprehensive geriatrician assessment as indicated.
Referral Criteria :
- Aged 60 and older
- Lives in the Vancouver area
- Has experienced a change in memory and thinking in the past 1 to 2 years
- Has insight into cognitive changes
- Is able and motivated to engage in self-management strategies
- No known cause of cognitive change (i.e. CVA)
Individuals who meet the outlined criteria, may self-refer to the clinic. A family doctor or nurse practitioner may also make a referral. Meeting with the specialist physician (geriatrician) requires a referral from a primary care practitioner. If you self-refer, and are eligible to meet with the geriatrician, they will need this referral to book you an appointment.
